Road M.A.P. for Success: Activities to Improve Blood Pressur
Blood pressure control rates dipped throughout the pandemic.  Implementing the MAP Framework can help reverse the trend.  This presentation will start with the American Heart Association's evidence-based strategies to improve the accuracy of blood pressure measurement.  Then a rural Wisconsin community health center will share how they have worked to address social determinants of health and empower patients in their own health journey.
