Join us in collaboration with Association of Asian Pacific Community Health Organizations (AAPCHO) for an engaging Lunch & Learn focused on the growing importance of addressing health-related social needs (HRSNs) at your health centers. This session will provide practical insights and tools around screening efforts and addressing challenges frequently.

During this webinar, participants will:
• Explore the importance of health-related social needs screenings and their impact on health outcomes,
         quality of care, and health equity
• Gain a general introduction to PRAPARE and other commonly used HRSN screening tools
• Learn strategies for implementing screenings in busy clinical environments while minimizing staff burden
• Discuss common challenges health centers face around implementing screenings and workflow concerns
• Explore practical approaches and promising practices to help health centers address barriers and build sustainable screening processes
