Innovations in Diabetes Translation Research
Accelerating Health Equity And Eliminating Diabetes Disparities in Community Health Centers (AHEAD-CHC) is a collaboration between the NIH/NIDDK-funded Chicago Center for Diabetes Translation Research (CCDTR), AllianceChicago (AC), MidWest Clinicians’ Network (MWCN), Health Choice Network (HCN), and the Clinical Directors Network (CDN) that serves as a hub to catalyze diabetes translation research in CHCs. AHEAD-CHC advances diabetes-related research and practice in CHCs through existing resources and services offered by the partner organizations, including a diabetes data repository, consultative services, and dissemination activities. This session will feature a panel presentation with representatives from AHEAD-CHC partner organizations discussing the innovative research that has grown out of this collaboration.

In support of improving patient care, this activity was planned and implemented by the Illinois Primary Health Care Association and Moses/Weitzman Health System and its Weitzman Institute and is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team. Additional CE credits are available for dentists, pharmacists, physician assistants/associates, psychologists, registered dietitians, and social workers.
