Electronic Medical Records (EMRs) can play a critical role in closing these gaps when used strategically and consistently. Part 1: Illinois FQHC Capitated Alternative Payment Methodology Speaker: Art Jones Part 1 will provide a brief update on national trends on the adoption of capitated FQHC APMs by FQHCs followed by a detailed description of the Illinois capitated APM. The information will help participants understand the opportunity APM offers and how it can be complementary to other value-based payment models they are already contracted for or contemplating. Part 2: Preparation for Adopting the APM & New Clinical Models of Care Speaker: Art Jones Part 2 will discuss preparation for adopting the APM. This webinar includes how FQHCs may want to set up the payer class options in their electronic health records, the importance of working with patients to appropriately change their primary care provide assignment, and how to modify their value-based payment contracting strategy with payers. Discuss new clinical models of care that become uniquely financially sustainable under capitation rather than fee-for-service. Part 3: Financial Modeling for the APM Speakers: Art Jones & Peter Ep Part 3 will be dedicated to understanding financial modeling for the APM. Peter Ep and Art Jones will review the newly updated APM modeling tool. Updates reflect the following changes in the model: • Calculating a separate rate for the three Medicaid eligibility types (e.g., temporary assistance for needy families, children’s health insurance program, Medicaid expansion, and the aged, blind and disabled) • Incorporation of a leakage factor Part 4: Q&A Session Moderated by Cyrus Winnett Questions from the audience and peer discussion about how they are evaluating the APM opportunity.
